Help us improve what we do.

We would like you to tell us what you think about the quality of care provided at the Nexus GP SuperClinic in Wallan.

If you would like to complete a Practice Accreditation & Improvement Survey ask one of our friendly Client Engagement Officers before or after your appointment. Participation in the survey is voluntary, you do not have to participate if you do not wish – non participation will not affect your treatment or relationship with our team. All feedback is confidential and anonymous – Nexus will not see individual results or comments.

Nexus is committed to quality improvement in everything we do – we value your participation in survey and feedback processes as this provides us with an opportunity to improve.

Extra Information:
– patients wishing to participate must be over the age of 16 years.
– the survey should take no longer than 3-5 minutes to complete.
– survey questions relate to the practice’s quality of service including the care provided by the clinician on the day.
– it is recommended to complete the survey immediately after the consultation – rather than taking it home.

Completed surveys should be placed in the sealed envelope provided and returned to the ballot box at the Nexus GP SuperClinic Reception.
