Nexus After Hours medical neighbourhood project launches

The Nexus Afterhours Medical Neighbourhood Project encompasses a community development approach to engage with vulnerable communities outside of the usual 9am – 5pm business hours in Wallan and Kinglake.

As part of our project we are working with the community to create spaces that are safe, inclusive and welcoming for all. The project will also increase home visits, access to Allied Health & Specialist Services, including Telehealth appointments & alternative therapies.

Rather than traveling to wait in busy Emergency Departments we will be available to meet your medical needs in your community.

Nexus will be promoting your medical neighbourhood through monthly activities.

Each month we will host a series of groups and events that support you to live well in your community.

This initiative is supported by Australian Government funding under the PHN Program.

Launch of the Nexus Primary Health Innovate Reconciliation Plan (RAP)

We have some exciting news to share! Our Innovate Reconciliation Action Plan (RAP) has been endorsed by Reconciliation Australia and we are proud to launch our plan to our community! Our Innovate RAP is an important document which focuses on developing and strengthening relationships with Aboriginal and Torres Strait Islander peoples, engaging staff and stakeholders in reconciliation, and developing and piloting innovative strategies to empower Aboriginal and Torres Strait Islander peoples.
