Continence Services

Nexus Continence Service helps people of all ages and abilities who have bladder and/or bowel issues, such as incontinence. Incontinence is when you unable to control your bladder, bowel or both your bladder and bowel. It can be a small amount or more severe.
 
Our specialised Continence Nurse will work with you. We can give you education to help you manage your incontinence. This may include:

• Your home life
• Work, school or volunteering.
• Outings and social events
• Exercise
• Travel
• Relationships and sex

Our Continence Nurse can help you in many ways, including:

• A continence assessment
• Recommending  different aids and equipment
• Making a plan to suit your needs
• Providing education and advice
• Referrals to other specialists or health services
• Give advice on funding options to help your management plan

We can help you stay healthy inside and out. Our goal is to help you stay active at home and in your community.

What to expect

Your appointment will be:

• At our centres
• Using telehealth
• Home Visit. We can come to you if you cannot visit us due to age, mobility, disability or access to transport.

Our team will discuss putting together a plan with you. We will make a plan that best suits you. We are able to provide advice to your care givers if that is needed.

Team

Our Continence Nurse Advisor is experienced and qualified, with:

• Post graduate studies in Continence and Prostate Nursing care
• Bowel care and management
• Qualified Sexual and Reproductive Health Nurse
• Qualified Paediatric Nurse
• Allied Health experience
• Disability sector experience
• Experience in the community setting
• Registered with AHPRA (Allied Health Practitioner Regulation Agency)

Referrals

You can be referred to us through:

• Your NDIS plan and ask for 'Support area: Improved daily living skills'
• Your Case Manager
• Home and Community Care (HACC)
• Department of Veterans' Affairs (DVA)
• Transport Accident Commission(TAC)
• GP Referral
• Self referral
• Carer referral
• Family referral

You will need a referral from My Aged Care if you are over 65 years old. They can be reached on:

• You will need to ask for Nexus as your preferred provider.
