Diabetes Education

Our Diabetes Education service wants to provide you with the best advice on diabetes management. We are interested in your long-term health and wellbeing. Our advice is based on current, evidence based information.

We can give support to all people with diabetes. It does not matter how long you have had diabetes, we can support you when:

  • You are newly diagnosed
  • You have not seen a diabetes educator recently
  • There is a change in your treatment
  • You have questions about diabetes
  • You feel like you need more help

We will work with you as part of  your health team, which may include:

  • Your GP
  • Your Endocrinologist
  • Allied Health
  • Other specialists or health care workers

We can provide education and support for you to:

  • Understand diabetes
  • Set and achieve your health goals
  • Monitor blood glucose levels, if needed
  • Understand and use Flash and Continuous Glucose Monitoring (CGM), if needed
  • Provide support with insulin pumps
  • Understand the role of  diabetes medications
  • Start and titrate insulin, if needed
  • Feel confident and supported in managing diabetes
  • Have a Sick Day Management plan – Know what to do if you are unwell
  • NDSS (National Diabetes Services Scheme) registration

Group Education can be a really helpful way for you to learn about Diabetes. Our regular education sessions include:

  • Diabetes Essentials: This group is for people with Type 2 Diabetes. It is for those newly diagnosed or as a valuable refresher. This runs every 2-3 weeks.
  • FlexIT (Flexible Insulin Therapy) - This is for people with Type 1 Diabetes, using basal bolus insulin.

We also run face to face NDSS programs, including:

  • CarbSmart
  • FootSmart
  • MedSmart
  • ShopSmart
  • Ready, Set, Go - Let's Move
  • Living with Insulin

What to expect

When we receive your referral we will call you to talk about the best education pathway for you.

In an individual appointment, we will ask you about your needs and talk to you about your health. You will need to bring information about your medications and blood glucose levels.

We have appointments available from Monday to Friday, with some after hour's appointment. Your appointment will be:

  • At our office
  • By Telehealth
  • We may be able to do a home visit if you are unable to come to our office. There are different fees for home visits.

Group education sessions are run in and outside of business hours. Call us for more information on our groups.
