Nexus Primary Health NDIS Support Coordination

Support Coordination provided by Nexus is focused on educating, empowering and connecting participants with compatible services to reach their NDIS stated goals.

Primarily focused on understanding the NDIS processes such as:

  • Your Plan sections, budgets and service agreements
  • Engaging supports from NDIS providers, informal and mainstream supports.
  • Working with Allied health professionals including referrals, reports and Assistive technology or the Home modifications processes.
  • Empowering you to prepare for a plan review.

Support Coordination is part of a Participants Capacity Building Budget in there plan. This section is about building independence and skills, so your support coordinator will be working with you, your career or advocate like a coach in a team to exercise choice and control to navigate and make the most out of your NDIS funds within your approved plan.
