Regional Assessment Services

The Regional Assessment Service provides assessment for people wanting to use the:

• Commonwealth Home Support Program (CHSP). This is a program for people over 65 years old.
• Home and Community Care program for younger people (HACC PYP). This is a program for people under 65 years old.

We will do a home assessment to see if you can use this program. You will need to meet criteria to get funding for services.

Our Assessor will discuss your eligibility to the programs when they do a home support assessment. You need to meet the criteria for these programs to get subsidised services.

We can make other referrals for you as well.

What to expect

We will make an appointment with you when you are free. We will come to your house or talk to you over the phone. Our assessor will talk to you over the phone. You can have a family member or friend with you when we assess you.

We will talk to you about the support you need. We will talk to you about if you are eligible for the subsidised services.

We have 5 assessors. Our assessors have experience in:

• Aged care
• Disability
• Mental health
  

Most of our staff live and work in rural Victoria.

Referral process

You will need a referral from My Aged Care if you are:

• Over 65 years old
• Over 50 years old and are Aboriginal or Torres Strait Islander
