Home and Community Care for Younger People (HACC-PYP) services include:
  • Assessment
  • Community Care: Domestic Assistance, Personal Care, Respite Care, Meals and Property Maintenance
  • Community Care: Allied Health and District Nursing Services (DNS)
  • Access and Support
  • Volunteer Coordination
  • Youth Outreach
This is a program for people who have a disability, injury or complex health issue, who are:
  • Under 65 years old
  • Under 50 years old if you are Aboriginal or Torres Strait Islander
  • And do not have NDIS (National Disability Insurance Scheme) funding.
This program is available in:
  • Mitchell Shire
  • Murrindindi Shire
  • Strathbogie Shire

Our assessors will visit you at your home after we receive your referral. We will talk to you about:
  • Your support needs
  • Your eligibility for the service
