Home and Community Care

Our Home and Community Care Services can help you stay independent in your own home for as long as possible. We will work with you, and those who support you, to understanding the things that are important to you. We will help you do as much as you can for yourself.

You can use our service if you live in:
  • Mitchell Shire
  • Murrindindi Shire
  • Strathbogie Shire
We can assist you with:
  • Your health, such as providing advice, information, or referring you to other services to stay fit and active.
  • Health eating. This can include using delivered meals, or showing you new ways to prepare your meals or assist with grocery shopping.
  • Getting out and about, such as being a member of social groups or helping you get to appointments.
  • Looking and feeling your best. This can include personal tasks like having a shower and getting dressed so that you remain in control.
  • Help around the house, such as keeping the floor tidy, making beds or doing the laundry. We also have a Home Maintenance service to help with jobs outside your house.
  • Respite so that carers of a family member can have a regular break.

What to expect

We will have an assessment with you. We will ask you questions to understand:
  • You
  • What you would like to continue to do
  • What you need help with to stay safe and well
We will make a plan together to help you stay living in your community. We will work together with other people who support you, like family, friends and your doctor so that you can get the most out of your plan.

We will help you use our services or other organisations if we cannot provide what you need. We can change the plan as your needs change.

We have over 70 Community Support Workers. All members of our team have:
  • Certificate III in Individual Support or a similar qualification
