Client Survey

In collaboration with our colleagues at Community Health First, we are currently asking past and present clients to take part in a short survey.

If you have used one of our services over the past year, we would like to hear from you about your experience.  Your answers are confidential.

As a thank you, you may choose to be included in a random selection to receive a $50 voucher after completing the survey.  The recipient will be contacted by phone.
