There are diverse range of services within our Community Support team that are there to support you to live well and independently in your community.

Our team of skilled professionals can work with you on personal problems that may arise from matters such as financial difficulties, family violence, alcohol and/or drugs, age or disability. Our team can work with you to achieve your goals by providing information, support, advocacy, and services such as Meals On Wheels and Home Maintenance to support you living at home for longer.

Access and Support

Access and Support is for older people who could benefit from community services, and who are disengaged from or are unsure about the services available to them because of their diversity characteristics.

We can assist you to break down the barriers that might have stopped you from accessing assistance in the past by ensuring that the services that you require are respectful and appropriate. We understand that there are many factors that might influence a person’s individual circumstances and needs, both positively and negatively.

Through Access and Support we can:
- Visit you at home,
- Listen to your concerns,
- Respect your views,
- Provide information,
- Offer choices,
- Arrange referrals,
- Advocate on your behalf.

Alcohol, Tobacco and Other Drugs

Confidential alcohol and other drug counselling, support and education. Our service is available to adults and young people if you live, work, study or spend a significant amount of time in Mitchell or Murrindindi Shires

Home Maintenance

Home Maintenance is part of a range of Home & Community Care Services provided by our team to help you, or the person you care for, to remain independent and in your own home for as long as possible.

We can assist you with:

• Help around the house – we can support you to maintain a safe household both inside and out. On the outside we offer basic home maintenance including window and gutter cleaning, minor pruning of overhanging tree limbs, home modifications such as grab rails or other equipment to support you.

Personal and Home Care

Personal & Home Care is part of a range of Home & Community Care Services provided by our team to help you, or the person you care for, to remain independent and in your own home for as long as possible.

We can assist you with:

·  Looking and feeling your best – our team will work with you to manage or modify personal tasks such as having a shower or getting dressed to ensure that you remain in control.

·  Healthy eating and food preparation – we can help you to develop and maintain a balanced and healthy diet by assisting you with tasks such as shopping & budgeting, and by introducing you to new meal preparation techniques that you might not have tried before.

• Help around the house – we can support you to maintain a safe household both inside and out. We can assist on the inside by keeping the floors tidy, making beds and completing the laundry.

Transport Assistance

Transport Assistance is part of a range of Home & Community Care Services provided by our team to help you, or the person you care for, to remain independent and in your own home for as long as possible.

We can assist you with:

·  Getting out and about – we can support you to remain active in your local community by maintaining membership of your preferred interest groups and/or social activities. We can help you to get to the places that you want to go including health-related appointments.
